Proof of Identity
You must provide one of the following unexpired documents:
- Valid Driver's License (U.S.)
- U.S. Passport or Passport Card
- State-issued ID Card
- U.S. Military ID
- Tribal ID Card
- Government Assistance Program Card (with name & photo)
Proof of Program
If qualifying by program, provide a document dated within the last 12 months:
- Benefit Award Letter (SNAP, Medicaid, SSI)
- Statement of Benefits
- Program Participation Card
Must show your name, program name, and date.
Proof of Income
If qualifying by income, provide documentation of your annual household income:
- Prior year's State, Federal, or Tribal Tax Return
- Paycheck stubs (3 consecutive months)
- Social Security Statement of Benefits
- Veterans Administration Statement of Benefits
- Unemployment/Worker's Comp Statement
